The following 3 forms are needed for completion of registration:
1) Medical Release Form: littleleague.org/downloads/medical-release-form/
2) Concussion Form: parents forms vball.pdf
3) Sport Parent Code of Conduct Form (both attached in link above with Concussion Form)
Please complete all 3 and return to your COACH. They will then provide all players completed forms to the board.
